I am looking for a good reference book or something to show some notes which can be picked when changing between chords...

For example, going from the G chord to the D chord, you can pck the C and then B on the 5th string then the open D 4th string...

I am looking to get ahold of these types of chord changes for all chords.
Obviously I would love to make it to a few jams and pick up the changes that way, but am out of the country for now.

Anyone know a good source for the chord changes? books, etc.
